I would guess that as long as the Republican Guard remains loyal to the regime i.e. they and their families have “bread” they (the clerics) will remain in power barring some outside direct military intervention. The trouble is that any replacement government would after taking over would very quickly target said Guard as the architects (likely) of the clerics decades long ability to terrorize the people. They (the Guards leadership especially) would have allot to answer for and they know it; their perceived only hope of staying alive and not imprisoned is to continue to support the clerics.
