  1. I’m a bit disappointed that TSR articles do not render properly in IE. Luckily, I have Firefox installed, and they render properly there, but in IE, there is no word wrap at all (or, that is, it initially wraps, but then completely unwraps once fully rendered). Not that you have any control over it, Rand, but just in case others notice the same problem.

    Somehow, I’m not terribly surprised at the way the story unfolded for AC-67, nor for NOAA-F. I’ve seen way too many “close enough for government work” examples (and subsequent failures) to feel comfortable accepting that as a proper response to anything anymore. I also agree with Wayne’s assessment of the value of having people in the same room, face-to-face, when dealing with complex issues and judgment calls, compared to making those same calls via teleconference.

  2. Odd, the TSR pages work fine for me running IE8.0.7600.xxxx on a Windows 7 laptop.

    Must say I never knew about the wind shear effect on Challenger. Very informative and I agree with Eleazer’s conclusions too.
