Rand, as you say no system is “perfect”. But I heard one of the recommendations Lufthansa was going to take was the “two persons in cockpit at all times rule”. This from US FAA regs apparently. That doesn’t mean *both* pilots at all times. But if one gets up for a bathroom break he/she must be replaced with another crew member (read flight attendant), before they can leave. Seems reasonable, since another *really bad* scenario here is the wild chance that a pilot/co-pilot could have a medical condition (read stroke/heart-attack) while alone in the cockpit with again, no way for other pilot to get back in!
Now you have to ask if the cabin crew are assessed as thoroughly psychologically as the flight crew, wouldn’t want to leave a nutty steward in the cockpit with only one member of the flight crew. . .
